Learning objectives
The aim of this exhibit is to review a wide variety of imaging findings in conventional radiography (CR),
computed tomography (CT) and magnetic resonance imaging (MRI) in the evaluation of gout in patients with longstanding disease.
In this study,
we will review retrospectively every patient with gout including CR,
CT and MRI from our data base of musculoskeletal pathology since 2004 until currently.
Background
INTRODUCTION
Gout is a disorder of purine metabolism characterized by deposits of monosodium urate monohydrate crystals (tophi) in joints,
bones,
tendons,
bursae,
and periarticular tissue caused by longstanding hyperuricaemia.
Gout is the most common inflammatory arthritis in developed countries (it affects 1-2% of adults),
with a prevalence much higher in men than in women and increases with age,
especially in patients older than 30 years.
In women it mainly develops after menopause because the fall in estrogen (which is uricosuric) causes upper serum urate concentrations....
Findings and procedure details
In this review,
we will describe the spectrum of usual and unusual imaging findings in the chronic gout in common and rare sites.
The most common sites of presentation are (in decreasing order of frequency) feet,
ankles,
knees,
hands and elbows.
IMAGING FINDINGS IN CR
Radiographs of affected joints have little utility in acute gouty arthritis because generally many patients will not manifest abnormalities radiographic until many years later.
After of several years of intermittent episodic arthritis,
patients that have chronic tophaceous gout will show...
Conclusion
The radiographic features of chronic tophaceous gout are characteristic,
and sometimes pathognomonic,
no matter what joint is involved.
Consequently radiologist must perfectly recognize imaging findings with CR,
CT and MRI in the evaluation of gout to make a more accurate and rapid diagnosis.
