Aims and objectives
BACKGROUND:
Presence Tracheal invasion (TI) alters the management plan.
Extensive local resection can improve survival rate and reduce local recurrenceVs.
near-total tumor excision with adjuvant treatment has survival rate similar to that obtained with extensive resection
Adjuvant therapy such as I-131 therapy and EBRT may fail to control the residual disease if significantly large
Hence,
assessment of extent of TI with pre-op imaging is crucial in appropriate treatment planning.
Although highly useful in the characterization of thyroid nodules,

Methods and materials
Retrospective review of clinical database at our center from Jan 2012 to Dec 2016,
104 PTC patients with extrathyroidal extension .
20 of these patients were excluded on the basis of the following criteria:inadequate information on the extent of disease intraoperatively and histopathology (15 patients),
Contrast CT examinations were not available (5 patients).
Final sample consisted of 84 patients
All patients underwent total or near-total resection of the primary tumor.

Results
Final sample consisted of 84 patients
19 men,
65 women;
age range,
19–84 years; mean,
54.2 ± 15.4 [SD] years
97 lesions (84 patients: one tumor in 71 patients and two tumors [one in each lobe] in 13 patients).
Mean maximum axial diameter of lesions was 3.5 ± 1.5 cm (one SD),
ranging from 1.1 to 8.4 cm.
80 patients underwent complete tumor resection and 4 patients had near total tumor resection.
There was substantial agreement between the two radiologists.

Conclusion
Presence or absence of tracheal invasion can be accurately diagnosed with CT imaging.
CT based Modified SHIN classification has a very high negative predictive value for predicting TI and can help optimizing postoperative outcomes with efficient preoperative assessment.
