To review non-tumoral complications of renal cysts.
Background
Renal cysts are sacs of fluid that form in the kidneys. Cysts can be located in parenchyma or adjacent to the pyelocalyceal system. They are usually defined as "simple" cysts, those with a thin wall and contain water-like fluid.
Renal cysts increased with age and usually do not cause symptoms.
Renal cysts are often found during imaging tests performed for other reasons. Some renal cysts may appear more complex and have a thicker wall or contain solid material instead of fluid.

Findings and procedure details
We present different cases of simple cysts that presented distinct complications such as Wunderlich syndrome (secondary to a spontaneous rupture of cyst in a kidney without prior injury and another in a kidney with acquired cystic kidney disease), retraction and partial emptying of a cyst after radiofrequency ablation of an adjacent tumor and infected cysts.
Wunderlich syndrome (WS) is a rare condition in which spontaneous nontraumatic renal hemorrhage occurs in the subcapsular and perirenal spaces.

Cysts are casual findings in imaging techniques that in a few cases can present complications such as bleeding, growth, urinary system compression, infection…
Radiologists should know the radiological features of complicated cysts to alert clinicians.
