Learning objectives
To review cat scratch disease (CSD) physiopathology, clinical presentations, and diagnosis;
To illustrate the most important sonographic findings in local self-limited and disseminated type of CSD;
To demonstrate the evolution of the disease through ultrasound follow-up evaluation.
Background
Cat scratch diseaseis typically a self-limited infectious condition, often present in children and adolescents, as they show high level of uninhibited interaction with pets than in adults. It usually manifests as a benign regional painful lymphadenitis that results from a cat scratch or bite involving the distal upper extremity.The initial trauma results in inoculation withBartonella henselae,a gram-negative bacillus,which cancause cutaneous lesions, such as a papule or pustule at the site of inoculation and subsequent regional lymph node enlargement, most commonly in the medial epitrochlear region,...
Findings and procedure details
We will present several cases and materials from our Pediatric Radiology group to illustrate the different clinical presentations of CSD and its main sonographic findings as well as the follow-up with sonographic evaluation.
1.Diagnostic procedures
Typical cases of cat scratch disease can be diagnosed based on clinical presentation and confirmed by serological markersused to detect a specific antibody forB. henselae(gold standard) or polymerase chain reaction (PCR). Biopsy of reactive lymph nodes although commonly performed is unreliable to establish the diagnosis of CSD but may be...
Conclusion
Recognition of cat-scratch disease is sometimes difficult because of its different forms of manifestation, from regional lymphadenitis to disseminated infection. Radiologists who are familiar with the imaging findings of this disease may suggest the diagnosis.The list of differential diagnoses for cat scratch disease is quite long and can delay treatment or may lead to erroneous management. Therefore, knowing the clinical history of contact with cats and main imaging findings of this disease is important to reach an accurate diagnosis and adequate management, although the definitive...
