Learning objectives
1. Case based approach in using multiparametric Spectral Data analysis in hepatic parenchymal ischemia.
2. Pictorial representation of the imaging findings on multiparametric assessment.
Background
BACKGROUND
Recent tertiary centre implantation of Spectral data analysis performed on the IQon Philips Spectral CT has the potential to revolutionize daily radiologist practice in the detection of acute thromboembolic pathology and the resultant end-organ parenchymal sequalae without the need for multiphase conventional CT. In doing so, there will be reduction in the acquisition times and radiation dose to the patient, potentiating improved turn over times and reducing patient risk whilst ensuring critical diagnoses are met without diagnostic uncertainty. The simultaneous analysis of Iodine density,...
Imaging findings OR Procedure details
CT demonstrates perfusional abnormality in the right lobe of the liver involving segment VI, VII and VIII. There is abrupt cut-off with segmental non opacification of the right portal vein and supplying distal branches. Monoenergetic low KV and iodine enhanced Z effective maps demonstrate increased perfusional defects in the Right hemiliver as a consequence. The iodine suppressed maps show no iodine within the right portal vein suggesting occlusion. These findings were confirmed sonographically. Following this, the patient was managed by the surgical team inpatient unit...
Conclusion
Multiparametric data analysis of the Spectral CT iQon and appropriate utilisation of Philips Magic Glass has the potential to improve the detection of parenchymal ischemia and vessel thrombosis using a monophasic, effiecient method in contradistinction to conventional CT, and the improved contrast resolution provides the potential for a more consistent diagnosis.
