Learning objectives
We will briefly review data regarding common areas of misses in neuroradiology studies based on our internal institutional quality improvement data. We will then present selected high-yield cases that were identified as either 1) “great calls” or 2) were missed on initial presentation. We will use these cases to illustrate challenges and pitfalls of clinical radiology. Each case will be followed by a discussion of how these challenges can be mitigated.
Background
Maintaining diagnostic accuracy is a fundamental goal in radiology. Institutional "great calls", where clinicians detect subtle pathology or demonstrate keen clinical judgment by synthesizing salient elements of a case, can likewise act as great learning opportunities. In our presentation, we share highlighted cases obtained from 4 years' worth of peer-learning data within the neuroradiology section at our institution. We expect that our findings are generalizable to a broad audience and can identify areas of challenge to facilitate self-improvement.

At our institution, we have an internal system for submission of neuroradiology cases as either “good calls” or “learning opportunities”. Selected cases are reviewed at regular intervals at our institution to facilitate quality improvement. This system functions as a non-punitive educational tool to provide feedback to attending radiologists and trainees alike, as well as to emphasize salient learning points from challenging cases in neuroradiology.

Conclusion
As the volume of medical imaging increases, the concern of missing critical pathology becomes more relevant. While some proportion of missed findings is unfortunately inevitable, we can learn valuable lessons from our misses as well as "great calls" where subtle findings were detected. Our presentation is a case-based approach to this topic based on 4 years of institutional quality improvement data submitted within our neuroradiology section.
