Long term results of percutaneous drainage and ethanol sclerotherapy in simple renal cysts with a different type of catheter

Background

Renal cyts are the most common benign lesions of the kidney .Unless pain and related symptoms are occured due to obstruction ,the cyts are required for the treatment. The first choice for the renal cyst treatment is percutaneous aspiration and scleroteraphy .Several different agents has been used for the treatment.

Between February 2000 till June 2011 in Baskent University School of Medicine ,Interventional Radiology Department we had done several cyst aspiration and etanol sclerotheraphy for the symptomatic cyst treatment.

Retrospectively we had searched for 132 patients .But 57 of them was excluded for the reason of not having enough clinical knowledges .

We had used only etanol as the sclerotheraphy agent .

The reduce in the volume of the cyst after treatment as % 60 was the initial criteria for the success.The reduce in the volume of the cyst after treatment %60-80 is called partial regresion and % 80 -100 was the complet regresion.We had done this criteria because all the patient was successfully treated .

The percutaneous cyst treatment and sclerotherapy is done for Bosniak cyst type 1.The percutaneous treatment is not done for cysts with internal septation ,thick walled or have calcifications .In other words cysts that is suspicious for malignancy is a contraindiciation for percutaneous treatment.

Several agents has been used for renal cyst aspiration and sclerotherapy.The agents are %99 ethanol ,OK-432,n-butyl cyanoacrylate ,iodised oil,povidone-iodine sclerosis,%50 acetic acid ,polidocanol ,ethanolamine oleate .The basic mechanism used for the agents is to destroy the epitelium tissue .However ,we used ethanol because it is safe ,not expensive and proven high success rate for the treatment.Also OK-432 is not found in Turkey .

Several different technics used for the treatment .Single session percutaneous drainage ,laparoscopic decortication ,laparoscopic unroofing ,finger asisted laparoscopic renal cyst excision ,retroperitoneal decortication ,percutenous marsupialization ,2hour and 4 hour retention ,three times repeated alcohol injections and drainage with nephrostomy tube can be considered.We used the single -session technic.

The patient history for the percutaneous treatment was pain ,hypertension and renal system dilatation .68 had pain ,4 had hypertension and 3 had renal system dilatation .After the procedure ,they had not any pain or related symptoms .
