Learning objectives
Review the main diseases that may present as pelvic acute emergencies in men and women.
Emphasize an approach by magnetic resonance (MR) imaging.
Background
Acute pelvic pain is a common symptom in the emergency department,
usually caused by gynecologic or urologic conditions.
An accurate and quick diagnosis is pivotal for the appropriate management.
Imaging assessment is an essential step on this conditions and the patients are frequently investigated by ultrasound (US) and computed tomography (CT).
However,
MR imaging has become increasingly interesting in this scenery,
especially for pregnants,
patients allergic to iodinated contrast or those who require reduced exposure to ionizing radiation such as children and young adults.

MRI imaging protocol
In general,
the main objective of the magnetic resonance protocol in the acute setting is to reduce the acquisition time without impairing the quality of the images.
This ismade by reducing the number of sequences obtained and individualizing the breath-hold techniques according to the individual's clinical condition.
Illustrative cases from our department were collected and classified according to these pattern types.
Clinical and radiology presentations,
differential diagnosis,
and teaching points were also described.

Conclusion
Although MR is less available,
its sensitivity and specificity in detecting the cause of pelvic pain are well established.
Therefore,
radiologists must become familiar with key imaging findings seen in urgent conditions,
as well as pitfalls that may lead to misdiagnosis.
