Learning objectives
Describe imaging findings in facet syndrome (FS).
Review the relevant anatomy of the posterior vertebral segment innervation for the interventional management of FS.
Explain and discuss different interventional techniques in FS treatment.
Background
Back pain,
especially in the lower segments affects a high percentage of the population,
representing the most common reason for consultation in primary care.
In developed countries around 70 to 80% of the population will suffer lumbalgia or lumbociatalgia with a peak incidence between 45 to 60 years,
representing a significant health problem with great socioeconomic impact,
high rates of absenteeism and sick leave.
Among the structural etiologies,
facet syndrome (FS) accounts for 15 to 45% of the cases of low back pain,
mainly due...
Findings and procedure details
Facet joints are symmetrical synovial-lined joints with a fibrous capsule.
They connect the articular facets of the vertebrae and are innervated by the medial branches of the dorsal rami of the spinal nerves.
Patients with FS may complain of low back pain that radiates to the thigh or iliac crest.
Exacerbation of pain commonly occurs during hyperextension,
torsion and lateral bending movements,
being it worse when getting up from bed or standing up after long sitting periods.
Initial management of these patients should be always...
Conclusion
Since FS is an important source of back pain and a major health problem specific interventional management is fundamental.
To achieve a better approach to FS knowledge of the anatomy,
imaging findings and different procedure details is essential for Radiologists.
