Learning objectives
To describe MR imaging features of the principal cerebellopontine angle (CPA) masses.
To illustrate the differential diagnosis according to radiologic-pathologic correlation.
Background
The cerebellopontine angle cistern is a triangular cerebrospinal fluid (CSF)-filled subarachnoid space bound by the pons, cerebellum and petrous temporale bone.
CPA masses represent 5-10% of all intracranial masses [1]; they are usually benign and divided into extra-axial and intra-axial tumors.
The principal subtypes are extra-axial tumors, like vestibular schwannomas (70-80%), meningiomas (5-12%) and epidermoid cysts (2-6%) [1] .
The other extra-axial lesions include arachnoid cysts, lipoma, aneurysm, etc.
Intra-axial tumors of CPAs are medulloblastoma, ependymoma and metastasis.
Clinical presentation of the CPA tumors is...
Findings and procedure details
Vestibular schwannomas
Vestibular schwannoma is a benign encapsulated tumor that arise from the Schwann cells of the nerve sheaths of cranial and spinal nerves.[5].
The majority (95%) of solitary lesions are sporadic. Bilateral vestibular schwannomas are highly suggestive of neurofibromatosis type 2 (NF2), a neurocutaneous disorder associated with a mutation of chromosome 22 [6].
Symptoms
The typical presentation is sensorineural unilateral hearing loss or non-pulsatile tinnitus. However, the symptom of vestibular schwannomas depends upon the size and location of the tumor: in the intracanalicular stage...
Conclusion
Lots of CPA pathologies present similar clinical symptoms, like vertigo or tinnitus, however, sometimes it is incidental findings.
Because each of theme leads to a different therapeutical pathway, a very good knowledge of the radiological anatomy and a specific MR patterns can help the radiologist to make the right diagnosis.
