Purpose
our study aims to evaluate the existence of a correlation between the presence of myocardial edema (indicated by the pathological hyperintensity in T2 STIR sequences), left ventricular function (assessed by post-processing analysis with Syngovia software), presence and extent of myocardial damage (evaluated with the Late Gadolinium Enhancment sequences) and risk of sudden cardiac death in patients with hypertrophic cardiomyopathy.
Methods and materials
67 Cardiac Magnetic Resonance Imaging (CMR) was retrospectively evaluated of Patients with Hypertrophic Cardiomyopathy undergoing treatment at the U.O.C. of Cardiology (Cardiomyopathy Clinic) of the "San Camillo Forlanini" Hospital in Rome, carried out in the period from January 2017 to October 2018 at the U.O. Cardiovascular Radiology (Cardioscience Department) of the A.O.: "San Camillo Forlanini" in Rome. The Cardiac Magnetic Resonance Imaging (CMR) were performed with 1.5 T Magnet (Siemens Magnetom Avanto), using the following study protocol:
• Cine - SSFP cardiac axes (2 chambers, 4...
Results
among the 67 Patients examined, 12 (18%) tested positive for myocardial edema; a significant reduction in the ejection fraction was observed in this subgroup compared to patients with negative T2-STIR (63% ± 6 vs 77% ± 10, p
Conclusion
The presence and extent of myocardial edema in HCM Patients shows a direct correlation with the degree of myocardial fibrosis and an inverse correlation with the ejection fraction.
A higher average frequency of TVNS episodes and a higher risk profile for SD are observed in patients with myocardial edema.
The presence of myocardial edema in HCM identifies a subgroup of individuals with a more unfavorable prognosis, with consequent implications for the follow-up and therapeutic management of these patients.
