Purpose
Immunohistochemical (IHC) markers, together with clinicopathological variables such as tumor size, tumor grade, nodal involvement, histologic type, and surgical margins, have been widely used for prognosis, prediction and treatment selection.(1)(2)
Correlating imaging phenotype with genomic information (often referred to as "radiogenomics") (3), in order to better understand genetic variability and the ability to predict prognosis or response to therapy, is a new field of research. (4)(5)
The 2011 St. Gallen panel supported the immunohistochemical determination of the estrogen receptor (ER), progesterone receptor (PR) and human...
Methods and materials
Patient population:Our study included 29 pathologically proven breast cancer cases who didn’t receive any treatment yet, with no general contraindications to MRI and good renal function, the patients’ age ranged from 29 to 65 years ( mean age ± SD = 44.2 ± 9.95 years) .
Data acquisition:
All MRI examinations were performed using 1.5 Tesla device, Conventional MRI study was done (including T1WI, T2WI and fat suppression sequence) as well as dynamic study upon injection of contrast media.Dynamic study consisted of one pre contrast...
Results
In our study, there were 29 malignant lesions; 16 lesions were positive ER, 20 lesions were positive PR and 15 lesions were positive HER2. (table 1).
There was a statistically significant higher proportion of positive ER and positive PR in mass lesions while non-mass enhancement lesions(fig. 1) showedhigher proportion of positive HER2(fig. 2) as detailed in (table 2).
Regarding the margins of mass lesions, HER2 was positive in half of the cases with irregular margin and in none of the cases with speculated margin. All...
Conclusion
Non mass enhancement lesions are more associated with HER2+ cancers, while the mass lesions are more associated with ER+ and/or PR+ cancers especially those with speculated margins. This may help in prediction of the type of tumor receptors within the lesion from its morphology-whether mass or non mass enhancement-affecting prognosis and management plans (the need for quick care, e.g. urgent biopsy and treatment protocol).
